Student Medical Documentation
Parents / Guardians of a student with medical needs, please follow these 2 simple steps:
STEP 1 - Complete & Submit Plan of Care. This form is to be completed for ALL students with medical conditions. Submit online OR complete, print/save as PDF & return to school.
STEP 2 - Drop off Student Medications & Medical Supplies/Equipment/Plan of Care PDF @ the school office.
